hi cindy, if u want the prilosec, just check the boxes that ask how often you have heartburn etc. like 4-6 times a week and u will get the sample. good luck
When answeing the questions you have to suffer from heartburn 2 or 3 times a week, if you answered 1-2 times a week you will not qualify.
Diana’s comment is correct.You have to suffer from heartburn 2 or 3 times a week. If you answered less times, you will not qualify. Good Luck!

Took me a few times it kept asking me to put in my first name and had to keep on doing it took about 5 times, but finally worked (on first click here) havent done walmart one (didnt notice it til now LOL)
